Getting started is easy
Schedule your first visit
If you haven't already, call the doctor's office listed on your patient letter to schedule your first visit. During your first visit, you can get your annual exam, talk
about your health goals and determine any specialty care you might need.

Prepare for your first visit
Get the most out of your first visit by preparing some basic information in advance.
While scheduling your first visit
- Ask about options for telephone or virtual video visits
- Ask how your primary care doctor's office can help you with transferring your medical record and existing prescriptions
Before your first visit
- Get to know your care network and resources available to you, like urgent care and hospital locations
- Make a list of any questions you would like to talk about with your doctor, including: How can I meet my health goals? How do I see a specialist?
What to do the day of your first visit
- Bring your medications, both prescription and over the counter, in their original bottles
- Bring your health insurance plan identification card and photo identification, such as a driver's license
